Leading on from the initial interview, this session can be a 'one off' or one of as many sessions as you feel you need..
This is where we form connections by getting to understand the symptoms, their origin and how this shapes a need for intervention.

Working towards your goals - overcoming barriers and problem solving
In the follow-up session(s) [as many as you would like and need], we explore at a deeper level what internal and external factors might be interfering with progress. We strive to make tasks easier by practicing effective strategies and forming workable routines; utilising available support and adjustments. This is where we 'make tracks', shape and form.

Evaluating change and adjustment - plan moving forward
At the end of any occupational therapy input, progress and adjustment to change should be reviewed. Often this involves the use of a standardised outcome measure which can be booked as final assessment and will produce a written summary report and longer term OT plan for recommendations. However, this evaluation session allows you verbal feedback and recommendations instead if you prefer.
